Agreement

I understand that when I give my e-mail address to Eastern Washington Center for the Deaf and Hard of Hearing, there is no privacy (or confidentiality). I agree to be told about emergencies and disasters by e-mail, TTY, or telephone.

 I understand that there is no guarantee (promise) that Eastern Washington Center for the Deaf and Hard of Hearing or other places will call or e-mail me in case of emergency, (Bad weather, fire, floods, bombing, etc.).

 I understand that Eastern Washington Center for the Deaf and Hard of Hearing and other places will decide when I need to know (be aware) of an emergency.

 I understand that I will allow Eastern Washington Center for the Deaf and Hard of Hearing and other emergency agencies to use my e-mail to find a good way to let me know about any emergencies.

 I understand that after I give my permission for my e-mail address to be used, I cannot take that back.

 I understand it is my responsibility to meet with a Deaf and Hard of Hearing specialist and the Eastern Washington Center for the Deaf and Hard of Hearing if I have any questions or concerns about this Emergency Notification Project.
